Dominion VITA Award

Dominion VITA Awards 2012 Nomination Form
Due by Tuesday, July 31 at 5pm.

Nomination Guidelines

1. Each organization can nominate one volunteer per year. If an organization has more than one volunteer committee (i.e., Board, Women's Committee, Youth Professionals, etc.), it is still limited to one nominee per year.

2. An organization with a winning volunteer must wait two years before submitting another volunteer nomination.

3. Sponsoring organizations are required to have a 501(c)3 status.

4. Nominees must be unpaid for their work with the sponsoring organization.

5. The nomination form is the only required material for consideration. Please do not include collateral material such as resumes, programs, etc.

6. Nominations for the Dominion VITA Award are not limited to adults. Youth volunteers in the arts are also encouraged to submit nominations.

7. Nominees are limited to organizations in Western Pennsylvania, Ohio, West Virginia and Maryland within the counties served by Dominion and WQED Multimedia.

8. Employees and Board members of Dominion and WQED Multimedia are not permitted to be nominated.

9. A panel of community leaders will judge the Dominion VITA Award nominations. Decisions are based solely on the information provided by the nominator.

10. The Dominion VITA Awards program is a joint community partnership between Dominion and WQED Pittsburgh. It is funded by an annual grant from Dominion. Dominion is one of the nation's largest fully integrated electric and natural gas holding companies. The Dominion VITA Awards are coordinated by WQED Pittsburgh, the nation's first community-owned public broadcasting station. Today, WQED Pittsburgh is a leading regional, public multimedia enterprise that creates and distributes quality products and services to engage, inform, educate and entertain the public within its community and around the world.
